MDTP - Send To Directors

Only Accredited K-12 schools and districts in California may order materials from MDTP regional offices.  Materials and test result reports must be shipped to school or district addresses.

Materials are provided without charge with the understanding that student responses (MDTP Answer Sheets) must be returned to MDTP for scoring. MDTP tests may not be scored by teachers, schools, or districts.

MDTP Diagnostic Test Reports will be sent to the students' current teachers. That teacher name must be on the class information sheet. Additional copies of reports may be requested for those with a legitimate need to the data, e.g., departmental, school, or district aggregated reports.

California Universities and Colleges, as well as Schools not in California, should not use this form. Please use the navigation links for additional information or e-mail mdtp@ucsd.edu.
