MDTP Answer Sheets

To help MDTP reduce its expenses, teachers are asked to only order the number of forms needed for the current testing session. Please allow for additional forms but do not order in groups of 500. Teachers using Edusoft or Datawise to administer paper tests should use answer sheets printed from those systems; they should not order MDTP answer sheets.

MDTP regional site offices ship and score MDTP Student Answer Sheets and Class Information Sheets to K-12 teachers, schools, and districts in California.  These valuable materials and services are provided at no cost to the schools or districts through the support of funding from the California Academic Partnership Program (CAPP).

MDTP offices are open year round and can provide materials and scoring services at any time.  Please send your order for materials as early as you can to ensure our offices have the materials ready when you need them.  If they have to restock or you order during a busy period then it may take from several days to 2 weeks to process your order.  You also need to allow time for shipping the materials to you.  You can order several months in advance and ask that they ship your order at a future date, e.g., 2 weeks before your anticipated testing date.

When submitting orders for materials or forms to be scored, please remember that MDTP asks that each teacher submits a separate class set for each class period that they teach.  Class sets should have a Class Information Sheet followed the the Student Answer Sheets.  The teacher name bubbled in on the Class Information Sheet should be the name of the students' current teacher; do not use  creative names.  The school name written on the Class Information Sheet should be the name of the school the student is currently enrolled in.  MDTP test results will be shipped to the teacher name at the school shown on the Class Information Sheet.

If MDTP offices have resources and staffing available, then upon request they can create aggregated reports for teachers, districts, and schools and then send the printed aggregated reports or send the electronic data to appropriate teachers or schools with a legitimate need..  Note:  Shipping and scoring student answer sheets and sending out  printed reports to the students' teachers takes priority over other services, as a result some sites may not be able to provide aggregated and electronic data during peak periods.Please see MDTP's Dissemination Policy.

Teachers should ensure that student names are bubbled in and there is no writing or marking by students in the timing marks. Please remove all unused sheets and scratch paper. Do not use staples, paper clips, rubber bands, colored paper, or other items to separate class sets.  Simply place class sets on top of each other and mail them in an appropriate sized package to minimize damage.  Class Information Sheets are also used to separate class sets.  One should be on the top of each set of student answer sheets.  The answer sheets have the top right corner notched.  Sites know to look at the right corner to determine where each Class Information Sheet are in the stack and each one begins a new class set.
